Medical Case Review:  Users of the Website have the opportunity of submitting their medical history and current medical condition(s) for review by Park East Gynecology & Surgery PC medical team and related staff.  If you opt to submit this form, you will be asked to provide a variety of personal information about yourself, including information that may be considered to be “protected health information.”  We will have access to all of the information that you choose to provide to us via this online form.

Contact Us: The Website provides a  “contact us” links for questions related to the medical services we provide.  If you contact us through this contact forms, we will collect your name, email address and any other personal information that you provide to us. We will use the information you provide to contact you back and to respond to any requests and/or questions.

Non-Personal Information We Collect by Automated Means And How We Use It:

We may collect certain non-personal information (data in a form that does not support a direct association with any specific person or individual) by automated means when you visit the Website.  Much of this information, when collected, will be through the use of third-party tracking services, which includes Google Analytics. Information about your computer hardware and software may be automatically collected by SEC.
